Selection Criteria Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) who is endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia. Reviews the patients with in the Residential Aged Care Facility (RACF) at GEDI/RaSS CNC or medical staff request, who would otherwise be sent to an Emergency Department. Reviews are supported by the provision of inpatient hospital standard acute care substitution within the RACF to those patients meeting appropriate criteria. Has a special interest in care of older persons. About the Team The Care in the Right Setting (CaRs) program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS Team includes Hospital in The Home (HiTH), Residential Aged Care Facility Support Service (RaSS) and Geriatric Emergency Department Intervention (GEDI) care providers. Patients receive quality care that is tailored to their individual health care needs. The HiTH service is a hospital substitution service with the expressed aim to provide medical, nursing and allied health care to patients in their home environment. Without a HiTH Service, eligible patients would otherwise be admitted to hospital for monitoring/treatment in a traditional inpatient bed. The RaSS model provides care in partnership with RACF's and GPs to improve patient choice of care setting and the quality and safety of care across the care continuum. About Your Role The Nurse Practitioner (NP) Oncology works in collaboration with the Multidisciplinary Oncology Team providing clinical expertise to patients and staff working within the MLHD-RCCC Oncology. Your Opportunity: The Nurse Practitioner (NP) will provide and coordinate comprehensive care of patients using advanced practice skills in the paediatric palliative care. The role of the Nurse Practitioner PPCS will: Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required. Provide safe, quality, cost effective care and practice within the Nurse Practitioner's approved Practice Scope that is consistent with contemporary standards and evidence-based practice.
